To make your tribute gift by mail, please provide the below following information with your honor/memorial gift:
• Honorarium or Memorial gift
• Name of person who is being honored by the gift
• Name and address of the honoree or family member so we may send an acknowledgment of your donation
• Your name and address
• How you would like your name listed in the card to the honoree or family
